Lifestyle Transitions can provide insight through an initial assessment of the person to identify areas of concern that lead to falls and increased dependence on others for self-care. Additionally, Lifestyle Transitions can assess the environment the person lives in to identify risks for falling, or how the environment either contributes to safety and independence or takes away from it.

OT evaluation: My rate is $250 per hour, with a minimum booking of 3 hours. This covers about 90 minutes of one-on-one service and an additional 90 minutes for report preparation, ensuring thorough and personalized care.

Traditional OT services in the home: 60-minute treatment: $400 (Ask about prompt pay cash discount)
