Michelle's consultations focus on evaluating your environment and training caregivers, not on providing clinical treatment.
Lifestyle Transitions conducts initial assessments to identify fall risks and dependency factors, and evaluates home environments for safety hazards and independence support.
Individuals & families (Care Transitions): $250/hour, 3-hour minimum — a minimum 90-minute in-home consultation plus a minimum 90-minute report preparation, with additional time billed at the same rate if report preparation runs long. Includes a comprehensive PDF report with photographs and recommendations.
Professional & institutional engagements (Professional Consultation Services): starting at $[X]/hour, scoped to the engagement — contact us for a proposal.
Beginning 2026, Lifestyle Transitions is no longer an in-network provider with any insurance, including Medicare, Medicaid, and private plans. All services — home safety assessments, care transition consulting, and professional consultations — are private pay only.
If your situation involves medically necessary occupational therapy treatment, we're happy to refer you to a covered provider. Our services focus on assessment, environmental modification, and caregiver training rather than billable clinical treatment.
Lifestyle Transitions serves North Texas, with no travel fees within Tarrant County. Beyond Tarrant County — please use the contact form rather than calling, so we can quote based on your location.
