Consulting to identify the “Next Steps” to assist in decisions about care when leaving the hospital, rehabilitation, or skilled nursing facility. Transition home safely through proper planning, such as outlining equipment needs or scheduling help with activities of daily living.
weekly/monthly check-ins
Maintain safety and independence through weekly or monthly in-person, phone, or virtual visits. This can include training for hired caregivers and consulting about daily routines and equipment that promote aging in place.
