As a mobile provider, Lifestyle Transitions comes to you to provide outpatient assessments and treatment traditionally provided in facilities connected to hospital clinics or free-standing clinics.
Lifestyle Transitions can help provide insight through an initial assessment of the person to identify areas of concern that lead to falls and increased dependence on others for self-care. Additionally, Lifestyle Transitions can assess the environment the person lives in to identify risks for falling, or how the environment either contributes to safety and independence or takes away from it.
Lifestyle Transitions can assist in evaluating how conditions or a diagnosis may impact living safely and independently at home before a planned surgery through PREhab or following a new diagnosis or discharge from the hospital. Conditions may include but are not limited to:
